The following principles embody my rhinoplasty philosophy and will be covered in the course by lecture and video demonstration:
- Secondary rhinoplasty deformities are not limitless: they form only three patterns. These three patterns can be anticipated and predicted in primary noses.
- Only four common anatomical deficits are responsible for these three deformity patterns. Each one can be diagnosed preoperatively from the nasal surface.
- Airway maintenance and correction should be part of each nasal surgery; septoplasty and valvular correction solve most airway problems. Airway reconstructive techniques will be presented.
- All key planning decisions can be made preoperatively from the nasal surface, so that the entire surgical plan can be set before going to the operating room.
- The great majority of rhinoplasty deformities can be corrected with only one of two operative strategies, using reduction techniques and four graft types.
- Case examples will demonstrate the endonasal reconstruction of primary and secondary deformities with septal, ear, and rib cartilage.
- Emphasis will be placed on diagnosis and directed, limited dissection that is effective, safe, and that minimizes the chance of iatrogenic deformities.
- Internal sutures, struts, and many of the graft types needed in open rhinoplasty are not necessary through the endonasal approach. Postoperative recovery is faster.
- Patient selection and ways of understanding and managing unhappy patients will be included.
